The Red Onion is awarded as a 'reward' when Melinoë selects the Vow of Forfeit in the Oath of the Unseen, replacing any Major Find she would have received in the region's first encounter. This includes rewards such as 
Boons, and 
Daedalus Hammer. The Red Onion is also given when you take damage when attempting the Unseen Sigil.
When consumed, the 
Red Onion restores a mere +1
.
